Programs
Medical Care
Each patient may retain their own physician who becomes a member of the Hospice team serving the patient and their family. Hospice has a team of certified Palliative Care Physicians and Nurse Practitioners to assume care for those who may not have a physician.
Nursing Care
Each patient will be provided a professional Primary Care Nurse trained in Palliative Care will visit wherever the person resides. The nurse will assess and collaborate with the physician to manage symptoms and maintain comfort. Pain management is a primary focus.
Personal Care
When the ability to care for oneself becomes difficult, Certified Nursing Assistants who are trained in Palliative Care are available to assist with bathing and other personal care.
Emotional Care
Licensed Social Workers and Counselors are professionals who will respond to questions and concerns honestly and with compassion. They will assist with emotionally coping with illness, care giving, stress, crises, conflict, grief, loss, and transitions in living arrangements. They will be able to coordinate resources to help the recipient remain home or make an acceptable choice of residence.
Spiritual Care
Spiritual Care encompasses the person's belief system, provides comfort, and contributes to a sense of well being. Through Spiritual Care, an individual may search and ultimately find meaning in life's challenges, and develop a sense of peace. Hospice Chaplains assist by listening, providing access to familiar rituals, facilitating an atmosphere to express feelings, and understanding the value of a life lived.
Volunteer Assistance
A Volunteer can be a source of support to the recipient or the caregiver by sitting with the recipient so the caregiver can shop or run errands, be someone to talk to during difficult times, and many times just a good listener.
